According to the chief, the root cause of the tragedy was a miscommunication issue between various utility contractors and their subcontractors. Abigail Barr, the wife of Cory Barr, the Sun Prairie Volunteer Fire Department Captain who lost his life in the blast, filed a lawsuit this week. In her legal claim, she named VC Tech, Bear Communications, USIC Locating Services, and WEC Energy Group (also known as We Energies) as entities responsible for her husband's death. The lawsuit seeks justice for the tragic loss of her spouse.

Two other volunteer firefighters, Ryan Welch and Greg Pavlik, have also initiated legal actions against these companies, citing personal injuries sustained during the explosion.

According to a recently released police report, the oversight that led to the unmarked gas line happened because one of the subcontractors had withdrawn from the project. Consequently, the company tasked with marking the pavement on behalf of the subcontractor failed to complete the task of identifying all underground utilities. When the job was transferred to a new subcontractor, the contractor working for Verizon instructed them to move forward under the assumption that the utility locating process had already been completed. However, this wasn’t the case. The new subcontractor accidentally struck the gas line while working in the street, triggering the explosion that obliterated several businesses and claimed the lives of Barr and injured several others.

The state Fire Marshal’s Office stated in its final report that they were unable to determine exactly what ignited the gas but confirmed that the explosion originated near the Barr House at 100 W. Main Street.
